MSK ultrasound volume increase higher among non-radiologists, study suggests

Between 2000 and 2009, the musculoskeletal (MSK) ultrasound volume increase among non-radiologists was much higher than that among radiologists, according to a study in the February issue of the Journal of the American College of Radiology. Ultrasound images of the MSK system provide pictures of muscles, tendons, ligaments, joints and soft tissue throughout the body.

Over the past two decades, MSK ultrasound has emerged as an effective means of diagnosing MSK pathologies. However, some insurance providers have expressed concern about increased MSK ultrasound utilization, possibly facilitated by the low cost and ready availability of ultrasound technology.

Data was obtained from the CMS Physician/Supplier Procedure Summary Master Files from 2000 to 2009, and records were extracted for procedures for extremity non-vascular ultrasound. Researchers analyzed annual volume by provider type using specialties, practice settings and geographic regions where the studies were performed.

Results showed that musculoskeletal ultrasound volume increased from 56,254 procedures in 2000 to 233,964 in 2009. However, the MSK ultrasound volume increase among non-radiologists, especially podiatrists, was far higher than that among radiologists from 2000 and 2009, with the highest growth in private offices.

"In a health care climate in which increased utilization deserves further scrutiny, this study has demonstrated significant utilization increases by specialties that are not traditional imaging providers and may be in a position to self-refer," said Richard E. Sharpe, MD, MBA, lead author of the study.

"These findings raise concern for self-referral," said Sharpe.

"Love, peace and soul." Don Cornelius would sign off with those words after every episode of his famed dance/music program "Soul Train," and now the television show host and producer is dead.

On Wednesday morning (February 1), Cornelius was found dead in his Los Angeles home on Mulholland Drive after police responded to a shooting call, the LAPD confirmed to MTV News. According to TMZ, Cornelius was found with a gunshot wound to the head. Sources told the website that Cornelius' death appears to be a suicide, however the LAPD could not confirm this information as the investigation is still ongoing.

Police discovered Cornelius' body at around 4 a.m. PT on Wednesday and he was later taken to an area hospital where he was pronounced dead. He was 75 years old.

Love, Peace and "Soul": Don Cornelius Remembered in Photos

A former radio disc jockey, Cornelius created, wrote and produced "Soul Train," a popular dance/music show which spotlighted soul, R&B, and later, rap music. "Soul Train" was patterned after Dick Clark's "American Bandstand," but it helped to shine a light on black artists who many times didn't get the recognition they deserved. James Brown, Aretha Franklin and Michael Jackson all appeared on the show as well as more contemporary artists like Chris Brown, Alicia Keys and Ludacris.

"Soul Train" was most famous for its dancers who would show off the newest moves while partying to the latest hits. The "Soul Train" line, where dancers would line up on the left and right of the dance floor, allowing for one performer at a time to shimmy down the aisle, became a staple in American pop culture. Cornelius would go on to host the show for 22 years, between 1971 and 1993. He eventually handed over the reins to new hosts. The show ultimately went off the air in 2006.

Share your condolences with Don Cornelius' friends and family, as well as you favorite "Soul Train" memories on our Facebook page.

LEBANON, N.H. -- Medical journal press releases are the most direct way that journals communicate with the news media about new research. According to a study in the British Medical Journal, press release quality appears to have an important effect on the quality of subsequent newspaper stories. With high quality press releases, key information basic study facts, quantification of results, acknowledgment of limitations is substantially more likely to appear in newspaper stories. Low quality press releases, those omitting such key information, may actually make newspaper stories worse than if there were no press release at all.

"The news media matter: medical reporting not only educates but also influences health beliefs and behaviors," said Steven Woloshin and Lisa Schwartz, principal co-investigators of the study. "Our study shows that press releases are important as well."

The investigators, co-directors of the Center for Medicine and the Media at The Dartmouth Institute of Health Policy and Clinical Practice, reviewed consecutive issues of five major medical journals that frequently receive news coverage but have different editorial practices. Two of the journals (Annals of Internal Medicine, Journal of the National Cancer Institute) include editorial notes highlighting study cautions in the studies, two do not (JAMA, British Medical Journal), and one journal in the group does not issue press releases (New England Journal of Medicine).

The authors reviewed consecutive issues of each medical journal (going backwards from January 2009) to identify 100 original research articles that generated newspaper coverage. A total of 759 newspaper stories (a median of three per journal article) were identified through searches of news article databases, Lexis Nexis and Factiva. The investigators did not examine broadcast, web and social media platforms.

All associated medical journal press releases a total of 68 were identified using the press release database http://www.eurekalert.org. Two independent research assistants assessed the quality of journal articles, press releases, and a stratified random sample of associated newspaper stories by using a structured coding scheme for the presence of specific quality measures: basic study facts, quantification of the main result, harms and limitations. In their analyses, the authors took into account whether the quality measures were available in the associated abstract of the medical journal article.

Of the 343 newspaper stories analyzed, 71 percent reported on articles for which medical journals had issued press releases. Some 9 percent of stories quantified the main result with absolute risks when this information was not in the press release, 53 percent did so when it was in the press release, and 20 percent when no press release was issued.

Of the total analyzed, 133 stories reported on research describing beneficial interventions. Some 24 percent mentioned harms (or specifically declared no harms) when harms were not mentioned in the press release; 68 percent when mentioned in the press release; and 36 percent when no press release was issued.

Some 256 stories reported on research with important limitations. Of these, 16 percent reported any limitations when limitations were not mentioned in the press release; 48 percent when mentioned in the press release, and 21 percent if no press release was issued.

The Dartmouth Institute investigators reported several limitations of their own study. They noted that since they did not conduct a randomized trial, it is possible that other factors besides the press release accounted for newspaper story quality. For example, medical journals that issue high quality press releases might take other steps that improve subsequent newspaper coverage (but they found no evidence for this). Other limitations: only newspaper print articles were analyzed even though web-based media is gaining in impact; subjectivity is inherent in any content analysis; and the investigators could have introduced bias in their use of stratified random sampling to select newspaper stories.

"Media coverage of medical research often fails to provide the information needed for the public to understand the findings and to decide whether to believe them. Although it is easy to blame journalists for poor quality reporting, problems with coverage could begin with the journalists' sources."

The investigators concluded that high quality press releases are a simple way for medical journals to increase the chance of newspapers reporting key information.

There is substantial room for improving press releases, the investigators noted. They believe medical journals should make the effort to do better and use press releases not simply to make medical news but to make news reporting better.

Bahrainis on hunger strike, official favors release (Reuters)

MANAMA (Reuters) ? Fourteen jailed opposition figures in Bahrain have gone on hunger strike ahead of the February 14 anniversary of a failed pro-democracy uprising, activists said on Tuesday, and a government official said he favored releasing some of the men.

"They demand an end to the political crackdown. They are protesting against the unfair trial they faced and they want the release of all prisoners of conscience," said Mohammed al-Mascati, head of the Bahrain Youth Society for Human Rights.

Mascati and several dozen other activists gathered at the opposition Waad party building in Manama to stage their own hunger strike in sympathy with the opposition leaders, who were prominent during four weeks of protests inspired by revolts against rulers in Tunisia and Egypt.

Bahrain imposed martial law in March last year and invited in troops from Saudi Arabia and the United Arab Emirates to help crush the month-old uprising.

The government said the island's majority Shi'ite Muslims had coordinated the protests with Iran for sectarian reasons, an accusation the opposition denied.

The 14, who activists said began their hunger strike on Sunday, were among 21 politicians, rights activists and bloggers tried in a military court on charges including "forming a terrorist group to change the constitution and its monarchical system" and organizing protests. Eight were sentenced to life imprisonment. Seven are abroad or in hiding.

Activists said the hunger strikers included rights activist Abdulhadi al-Khawaja, Al-Haq opposition party leader Hassan Mushaimaa and Ibrahim Sharif, a Sunni Muslim opposition leader.

Sharif's wife Farida Ismail said prison authorities had removed privileges such as access to television to try to force the men to end the hunger strike in Jau prison. She said they were being force fed.

"These trials were political, they were just revenge by the government," she said at Waad headquarters.

An interior ministry statement said the hunger strikers were being properly cared for. "While it is regrettable that this action may cause additional hardship for the detainees or put their health at risk, they do have the right to refuse food," it said, citing general inspector Ibrahim Habib.

"All of the inmates have been provided with regular medical care that is available to them 24 hours a day."

CONFESSIONS UNDER TORTURE

A rights commission on the unrest reported in November that detainees had been tortured. It criticized military trials and advised the authorities to have jail sentences reviewed.

The government, under outside pressure to implement the recommendations, has said a judicial panel will review some sentences. But they have not questioned the military verdict against the 21 protest leaders, who have the right to take the case to the cassation court, the highest appeal court.

A government official expressed hope some of the jailed protest leaders would be freed but said others had planned an Islamist coup.

"I am hopeful that a lot of the cases will be reviewed, but there are some cases to go through and cases have been transferred to the civilian courts," said Sheikh Abdul-Aziz bin Mubarak al-Khalifa, a senior adviser at the Information Affairs Authority and former ambassador to London.

"I'm hopeful for not necessarily all of them, but at least some of them ... There are those in prison who called for a restructuring of the country's institutions, for a full-blown revolution and who called for an Islamic republic using non-peaceful methods," he said.

Al-Haq and two other parties, Wafa and the Freedom Movement, formed a "Coalition for a Republic" during the protests that called for the creation of a democratic republic.

Bahrain remains in turmoil with daily clashes in Shi'ite towns and villages between protesters and riot police that have become more violent in recent weeks. The economy of Bahrain, a banking and tourism hub, has been shaken by the unrest.

The daily al-Ayyam - owned by a media adviser to the king - reported on Tuesday that contacts had begun with unnamed political forces for a new national dialogue "to bring Bahrainis together and strengthen national unity."

Opposition politicians said no one had contacted them and the report was window-dressing ahead of the February 14 anniversary.

Since the uprising, the government has strengthened parliament's power to monitor the cabinet, but has not reacted to opposition demands for a fully empowered elected parliament able to form a government.
